Convert 38 page PDF file into HTML website

PDF file is around 38 pages. It's a directory I created but I want an html version. I will include graphics so they do not need to be extracted. Other than possible resizing, there is NO photo manipulation. There's a screenshot for each web page listed in the directory. The PDF file contains hyperlinks which need to be including in the HTML site. Home page will contain basically the document's TOC. I will also need a simple header graphic. No need for meta tags, alt tags or any SEO. I just need a site the looks like the PDF document but functions as a website. One exception, I will also need a table containing the site's main categories (hyperlinks) that will need to be on every page for easy website navigation.
