Digital Check-In App and ADMIN Tool Solution (Web-app, android?)

I have a business where people come in groups to play a live escape game: [login to view URL] When people arrive they have to read some rules and sign them on paper. Then we transfer names and other data into an excel sheet. Later we export email adresses and import them in mailchimp to send emails. Further we put pictures of teams in Facebook album. I want to do this via a tablet (entry) and instead of the excel sheet I want an ADMIN tool where I can add additional data and do custom queries or see charts. The CHECK-IN-APP should: - consist of multiple pages asking a single question per page. the answers need to be clicked or entered Example: 1. Page: How big is your group: 2 - 3 - 4 - 5 - 6 [login to view URL]; How did you hear about us: google - newspaper - advertisment... [login to view URL]: What is your name: input field 4. Page: What is your email?: input field 5. Page: Please sign that you accept the rules: input field SIGNATURE with finger In the above example the tablet would be given to the next player of the group after the first has entered the data. The next teamember only has to fill in starting from page 3....until the last member closes the team check in. - the app should then be connected to a database ADMIN-TOOL a REPORTING TOOL (either via app or website ) needs to be build to analyse the data (show chart of age, country, group size. - a FRONT END for additional data entry and data analysis through queries or selections. Additional Addons: An API for Mailchimp should be implemented, so the emails and names can be put in the mailchimp account. (this is an addon and can be done in a second stage) Maybe Facebook-Integration so we can take pictures and they will be posted in a specific album on the facebook page. This combo-solution should be possible to adjust easy for other locations/branches of our business or even change for other businesses.
