Convert a manuscript into an eBook with InDesign

I need someone to convert my book manuscript - source is either a word document or a pdf file (about 560 respectively 456 pages) to an epub format with active table of contents (passive TOC provided) and cover image (provided). It would be great if you could also provide the Amazon Kindle mobi format in the same go. Don’t bid if you’re not capable of doing this professionally (use Adobe InDesign). This should not take long as the pdf is professionally formatted and ready to print.
