Attached are 6 questions.
I need a form creating which has the 6 questions listed out, with the answers.
Only one answer can be checked and each question needs to be answered.
Each answer has a numerical value and when finished, the form adds up the score and then produces a standard report which is emailed to the subject, only in return for an email address at this stage.
The email is to be submitted to aweber.
The form is therefore slightly intelligent as the correct answer needs to be sent depending on their "score"
The form will take on a simple look and will be hosted on my server.
