Web GIS With Spatial Database

Hello , I am looking for someone with GIS, Spatial Database, Google Maps experience. I need to be able to upload a csv or text file via a webpage and the location points are displayed on a map of the United States. See example below:

Fields to be used in import file will be: City, State, Zip
