Icon needed for an App

I require an icon made for a new application. This icon should conform to the apple guidelines as detailed in this url :-

Specifically I will need these icon sizes :-
120 x 120
1024 x 1024
I would want the original image layers , either PSD or a layered Illustrator file, together with the necessary jpegs.
As this is for an application, I would expect the design to be unique and not to be used for other projects by yourself.
There may be repeat work dependent on how this exercise goes. Please let me know examples you have done in the past when bidding.
