Create a simple form creator

I need a form creator created. You should be able to make a title and subtitle, and add fields (text fields, radios, checkboxes, upload files, etc.) to the form. You should then be able to edit the text on the submit button. Then, you should be able to input the email you would like to receive the results from (files would be sent as attachments). At the bottom of the page, you should be able to click 'create'. When you click create, a link will appear where you can view and fill out and submit the form. The link will be local: for example the script is uploaded at [login to view URL], the form should be at [login to view URL] The completed form should be very simple with a title and subtitle, all of the fields, and the submit button centred on the page.

This must be created in under two days. Please include the word "camerida" in your bid so I know you've read and understand the project.
