Small design for a newsletter signup sticky widget

If you visit [login to view URL] you will see a sticky footer which says "Free Pretzel Bites if you join our eClub" - I would like to hire someone who will create 3 versions of the same widget.
You are not designing a webpage or a full footer:
Just one line of text which will say:
"Join our rewards program to get a free cookie" and next to it will be a text field with a label called "Email" and a dropdown which will have a label of "Location" and use SUBMIT for the button.
Client wants to see a background color of #0774b5(blue) in use for one version, #b3be31(green) for the 2nd version's background and for the third, a gradient like the one in mellow mushroom's site will do.
Using white for the font color sounds good but for the button, on the blue background lets try the button in green and for the green background lets use blue as the button color. Please make the button and the text look modern like in mellow mushroom example.
