Build a Website

I need a form that is developed using AngulaJS,Javascript,Bootstrap.

form fields:

Group Name: (input field)

description: (input field)

Sent by; (input field)

defaultCC: (input field)

group disabled: checkbox(yes/no)

add record(submit button)

1)once the user clicks the submit button, the form data filled by the user has to be stored in Database table(Rapid SQL).

2)Group Name,description,Sent by is required fields

3)submit button has to be disable until the user filled the required fields. once the required fields are filled the submit button must be enable or available for the user to click the add record button.
