Incorporate our logo PEARLY WHITES EXPRESS (attached) in your design. 4. Design form that will collect name + email address configured with our MailChimp account then redirect to our checkout page (I will give you web URL that links to our [login to view URL] payment page) *You are not responsible for building our checkout system - just add a link that will redirect buyer after they complete form (opt-in to mailchimp list) *You are not responsible for video.
