1. Create a new database on your flash drive named quiz. 2. Create a New Table with the following fields and field types: Field Name Data Type Additional Specifications Fname Text Caption First Name Lname Text Limit last Name to 3 letters Age Number Primary key No Default value Phone Number Text Include input mask Birthday Date/Time Format Long Date, Input Mask Medium Date gpa Number Default value of 3.75 Numbers should be formatted for double precision floating point numbers with 2 digits after the decimal point. 3. Save this table and name the table for Your Name (substitute your actual name for Your name). 4. Change to datasheet view and put an entry in the table for yourself. 5. Add the following entries to the table. fName lName Age Phone Number Birthday gpa Goofy Dog 10 (111) 111 -1111 October 3, 2004 2.98 Mickey Mouse 100 (222) 222 – 2222 April 15, 1990 3.25 6. Make sure that all information is visible in the datasheet view of your table and save your table. 7. Setup the page to be printed in Landscape and print out your table. ] 8. Create a Form for this table. The form should have a style, a tabular layout, and should be named form for Your Name. 9. Save your Form and print it. 10. Close the database that you just created. Part II. States For this problem use the database states. This database only includes one table, USstates. Your job is to create each of the following queries and print out the data view (the dynaset) of each query. 1. Create a query that shows all states with nicknames starting with Land and all states with nicknames starting with Old. Put the states in ascending order by state. Name this query nicknames land and old for Your Name. 2. Create a query showing all states in the Mountain region with a population of at least one million. Put the states in reverse order of the year founded. Name this query mountain population more than one million for Your Name. 3. Create a query that shows all states whose name starts with M or N and whose population is between 500,000 and 3 million. Name this query m or n states for Your Name. Submit a print out of the datasheet view (dynaset) and a print screen of the design view (Print Now) for each of the 6 queries with your homework. (Be sure that the columns are wide enough for to display all information in your design view.) Part III. Employee For this problem use the file emp. You will be working exclusively with the Employee table within this database. 1. Add yourself to the table. Your business location is Sterling, VA. Your title is Account Rep. Your salary is $100,000. 2. Create a report that includes all employees grouped by location and within location grouped by job title. Include the average salary for each title at each location. 3.
