Simple Web Based Form producing XML output(repost)

I am looking for someone to write software for a handheld device. The software needs to work on a Windows Mobile Device, Palm and possibly Iphone.

The most straightforward method would be to write it for use in a browser. However the software should also be able to be used offline.

Currently we fill out a paper form then enter this into desktop software.

The desktop software will take an XML input but it has to be in a specific format.

I would like the software written here to run on a handheld device then either email or save locally an XML file than be read into the desktop software.

Attached are some sample files:

DEAP Survey Guide Vesion [url removed, login to view] - Last four pages of this are the paper form our users currently fill out.

[url removed, login to view] - this contains the XML mappings. All of the names of every XML entry that needs to be in the final outputted file including some brief descriptions.

[url removed, login to view] - The desktop software that will read in the XML file. This can be used to test the outputted file from the software.

Finally, the software should be secure, that is, have a username/password for each user that we (as admins) can setup.

Summary:

Software outputs an XML file that will open in the provided software ([url removed, login to view]). File is saved locally or emailed.

Software runs on Windows Mobile and Palm (required) and optionally on other handheld devices (such as the Iphone).

Software can run without an internet connection.

Software is secure (username/password).
