Code to add ticked check boxes in PDF document

We need a coder to write a script (Java script?) to add a number of check boxes in a PDF document. The document is a workbook used by students at a school. Students fill in the PDF document, the completed document goes to the teacher who ticks the check boxes next to the students answers (see attached document page 5) We need the ticks for each excercise marked to be totaled and put into the yellow text box (see page 5) Additionally we need code to add all the sub-totals to a final total for the document. We need to be able to reuse the code easily for other documents. Note we are using Omnipage PDF converter to add the PDF data elements (if thats relevant)
