I have a form set up for the person in my running club who organises the volunteers.
The form is called [url removed, login to view] and asks for:
stri_event and stri_date which is the name of the event and the date on which that event will occur. These can both be entered as text, and there's no problem with these fields.
Next I have 5 fields in which volunteer roles are to be listed. What I would like is the ability to add a number of fields (similar to the way fields can be added to a table on phpMyAdmin (I've set up the textbox and the "Go" button for this, but haven't added the functionality.
Then when the form is submitted, it is processed by the [url removed, login to view] file.
Basically I'd like whatever volunteer roles are put into the volunteer fields, to go into the database in the vol_role field.
So, if a person added an event called "Race 1" on a date "Monday, 1 Jan, 2011", and had 3 volunteer roles: "Pre-race set up", "Course marshall" and "Post-race pack up", I'd like that sent to the database in 3 rows.
Each of the 3 rows would have "Race 1" in the "stri_event" field and "Monday, 1 Jan, 2011" in the "event_date" field. Row 1 would have "Pre race set up" in the vol_role field; row 2 would have "Course marshall" in the vol_role field, and row 3 would have "Post-race pack up" in this field.
There's also a picture of my database if my explanation isn't good enough.
