Registration form errors

I have a registration form (in php) that sends completed data to a mysql database. The data is currently being sent and stored correctly.
There are 5 fields in total on the registration form.
Username, Email, Verify Email, Choose password, Verify password and all get corrected entered in the database but the form is not checking all fields are complete before sending.
Due to using a few unreliable developers the code has been changed several times. I am not looking for the whole registration to be redone just for code to be checked and corrected to check all 5 fields are completed before allowing the form to be submitted.
