Web page Staff time sheet Logging

I have a company that employee sub contractor full time and casual staff, the problem we have is they alway are late in either sending in invoices or hours of work. What I am after is a web page that the staff can log on to and fill in there password and name and get onand fill in there time sheets the time sheets need to be in two week blocks from wed to tuesday over a twu week period. The information that will be kept in the tables on there time sheets should have
Staff information
Date entrered
Date worked
Start time
Finish time
Length of Break Times
Job Site
Job name
Site superviser
Office use for approval
superviser Person aproviing times
Aproved start time
Aproved finish time
Total number oh hours worked
Once the time has been approved by superviser the staff member for that two week period can not change it after the superviser has approved
Then there need to be a web page for the Pay role staff member to call back the information on the dates that the pays are to be made up.

## Deliverables
1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.
2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables):
a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment.

(No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement).
