I need a loan application form. Please see attached example for fields/layout.
Developer Requirements
1. Form to be incorporated into WordPress theme (use same fonts, colors, etc.)
2. Required fields: borrower name, phone number, email
3. Validate borrower email address
4. Provide tabbing through form
5. Each section of the form should have a border similar to that shown above
6. Form to be submitted via email. Developer is responsible for determining website host requirements for submitting email (website hosted at HostGator). Email should be formated with questions/responses as above.
7. Once form is submitted, redirect borrower to a thank you/additional information page
8. Send borrower an email with additional required information and a copy of information submitted. Email should be formated with questions/responses as above
9. Submit Loan Application button similar to button above. Color to match website
10. Developer responsible for installing form and configuring the custom form mail, cgiemail
