magento CE 1.7 - new trade registration form that duplicates standard form

When a user on the front end of magento clicks onto 'register' you are directed to this page : customer/account/create/...........
we want to duplicate 2nd page with extra fields as maybe : customer/account/create/trade...........
the new registeration page will have extra fields and we need an email to notify us that someone has registered for a trade account:
*First Name
*Last Name
*Company
*Email Address
*Address
*Town/City
County/State
*Zip/Postal Code
*Country
*Telephone
VAT number
Company Reg ID
*Business type (new attribute drop down with admin for options thought to be - Interior Designers, Specifiers, Architects, Hotels, Restaurants, other)
new customer accounts created will be default GROUP and a new customer attribute 'request trade' (yes/no) need to set as yes and shown on the admin grid view 'Manage Customers'
as always -
you must be 100% magento specialist and not changing core files.
you must have a high completion score
you must have at least 20 projects
maybe this would be rolled up into an extension ?
