Add custom fields to Extensions page on FreePBX ver 2.11

We need a few custom fields added to the Extensions page on FreePBX version 2.11. Specifically, we need these fields added: Date of Activation, Rate Code, Address, and Notes.
So, when we activate a customer, we can populate those fields for our own reference. And when we use the Bulk Extensions to download the database, we need to see those new fields and the data. Conversely, when we modify the Bulk Extensions and upload, those fields are correctly mapped with the new data. There is no Asterisk or SIP integration at all- just the database and Extensions page integration. A screen shot of what we're after is included.
