Build a test application

We need a simple web application for testing candidates applying for test automation.
This will be a simple one page website for restaurant booking, where the user will enter their name, date of reservation(datepicker), number of guests(dropdown), payment method (dropdown). Creating, viewing, filtering and deleting should all happen on the same page.
We want you to use Angular.js so the input fields will have no obvious identifiers and candidates will need to work how to implement automated tests.
User will be able to delete individual records or all (in which case a confirmation is required).
And, on creating records, introduce artificial delay of 30s so we can see how candidates handle Webdriver waiting times
Design is not important, just working application required. The backend should be either Java or PHP with MySQL. We should be able to spin off an instance for each candidate (this will be done either manually or by candidate registration system), for example if the website is [login to view URL], then a candidate area will be [login to view URL] so they can only see the data they have entered themselves.
