Create an Online Retirement Calculator

I need an online Retirement calculator created from scratch.

(but I want it to look more modern)

The first page the user sees should be a page that has the following info on it:

Calculator Directions

* The calculator will ask you 7 questions.

* There are no right answers so please be honest with yourself. It won’t serve you to try and “beat the calculator” by answering in a way that will make you look the best.

* The calculator will take around 60 seconds to complete and you’ll get your results and customized advice at the end.

Along with a button that takes them to the first page of the calculator.

The calculator should ask 7 questions (each question on a separate page):

1. Are you male or female? (include 2 radio buttons and when one radio button is clicked, it should go to the next question)

2. How old are you? (text box that only let’s the user enter a 2 digit number)

3. What age do you want to retire? (text box that only let’s the user enter a 2 digit number)

4. How much do you currently have in your retirement accounts combined (IRA, 401k, savings, etc.)? (text box that only let’s the user enter a string of numbers)

5. What yearly return are you getting on your retirement funds right now (%)? (text box that only let’s the user enter a 1 or 2 digit number)

6. How much do you contribute to your retirement accounts every year? (text box that let’s the user enter a string of numbers)

7. How much money do you want to spend every month during your retirement? (text box that let’s the user enter a string of numbers)

After they answer the 7 questions, they should be sent to a page that asks for their first name and email address. I will provide the form code for this. But you must also use the data submitted to send the user an email with their results (basically the same text they will see on the thank you page listed below). The email should also note the user’s IP address.

At the bottom of the email it should also say:
