Create four WordPress Forms/Log-in pages and capture data to iContact on existing website

Summary Design and develop on an existing WordPress website ([login to view URL]) two separate resource pages each with their own secure log-in. The pages will have training program participant resources such as PDF's, videos and Audio Clips. Design and develop one 'Get a quote" form and amend the "Contact Us" form _____________________________________________ Consider: Design using existing website style (or improved style) so that the look and feel is professional and smart looking. Link all 4 forms to my existing iContact database (develop coding page available) Details of the 4 projects are listed below. I would also like your advice on any features or designs that would enhance the projects below. I will also be posting a project for a small site update so let me know if you are also good at design and style. _______________________________________________ 1) Form 1 - Log-in to secure Participant Page Create email and password log in and a sign up page Include: Log-in - Email - Password - Forgot Password - Remember me - Sign up Sign up form has the following information: • First Name • Last Name • Email • Password • Name of program attended • Business Name Provide advice on approval options (automatic approval vs website admin approval). It would be good to have the option to use the "Name of program attended" field as a pass code - if they add in the word "DiSC" it automatically logs them in - if they don’t it would require Administrator approval (ie be delayed)? Capture sign in content info into Client Content Management Application (iContact [login to view URL]) Design and Development of Resource page layout so I can attach audio clips, videos and PDF resources. Advice on WordPress storage space and other storage ideas (such as video hosted on Vimeo) as some of the clip are large file sizes. Once logged in they would also have access to the website visitor page below (separate page) ______________________________________________________ 2) Form 2 - Website Visitor Call to Action Log in page This would be similar to the above page but be a second resource page with limited resources. A sign up bar would be located on each of my website pages (above the existing "Book a Program" form) Text: "Sign up for a copy of each profile and other free leader and team development resources" Similar sign up page to above with: *First Name *Last Name *Email *Password Address 1 Address 2 City State Postcode Business Name *Mandatory Capture sign in content info into Client Content Management Application (iContact [login to view URL]) Design and Development of Resource page layout so I can attach PDF resources. ________________________________________________________ 3) Form 3 - 'Book a Program' form amendments. This is an existing form on each page of my website. I'm currently using "Wufoo" for this form and am almost out of my free 100 for uses. I'm looking to transfer this form to another type of platform that is the same as the platforms above. This form is on most of the website pages so any change will need to be changed on the 10-20 pages in the WordPress back end. _____________________________________________________ 4) Form 4 - 'Get a quote' - same questions as the 'Book a program" form but add a free text box at bottom with "Details of enquiry" I'd suggest that this form be on the same page as the "Contact Us" form (maybe divided the page in half - Clean up existing 'Contact Us" form (eg reduce length of fill in boxes) and add the form under the current 'Get a quote heading" Suggested text would be: Want someone to assist you with program ideas? "Contact Us" Know what you want and need a quick quote "Get a quote" Both forms would capture data into iContact database as above. Add 'Contact Information' currently on this page (address etc) to top of page or in a side banner
