I've been working with Magento v1.x since version 1.7. Items 2,3 and 4 described can be done within 2 days. The first item in other 2 days, but I advice to change the part that says "When you select this checkbox it should reload the page and create the {date created} " to creating the data when saving the order with the checkbox marked. The reason why I say that is in my long experience in Magento, I very often undo changes before savings products, categories, attributes and orders. Another reason is that it would be much faster, because magento it self was built with an structure that makes it faster to be done. My offer would be $250 for 1 week of work. I put 1 week so that I don't miss the deadline, but 90% it'll be faster than that.
