A Simple CRM Software

I need a simple CRM software, which will run on a server and users can login the server via web interface.
Software has to store data, get data and search data.
I have 3 main entities to store their data: Customer, Potential Customer, Coder
Customer has following fields:
Company Name
Company Sector
Address
Phone
Tax Office
Billing Address
Contact Name
Referrer Name
Order
Due Date
Assigned Coders
Description(I need an auto date appended time when a description written down)
Notes
Coder has following fields:
Name
Address
Phone
Price
Bank Account Details
Potential Customer has following fields:
Company Name
Company Sector
Address
Phone
So I need a database designed to capture these entities in MySql, and a web gui to store, get and search these data. I hope I stated the problem clearly. If you have any questions please ask. And also you can make suggestions about changing programming languages or database, but I think PHP and MySql suits well.
Notes: I want the source codes as well.
