Create htaccess file, fix site redirecting to sub page instead of main page

I need a simple htaccess page created to forward 404s to my custom 404 page. Also, all of a sudden my main site page is not loading, something happened on the server. the site, [login to view URL], is now going to my [login to view URL] page, a phpbb3 instead of my normal [login to view URL] page. need this fixed asap please.
