cubecart modification on checkout (or All in one Shipping extension)

Hello,
I need to add 2 additional drop down boxes under checkout in my cubecart installation.
Those boxes should display upon checking the option "delivery", we currently have "delivery" (using All in One shipping extension) or "pick up" (using "store collections" module) options)
If "delivery" is selected, I need to lookup the shipto zipcode and match it to a separate table in mysql with the following structure:
date1 time1 timeframe slots count zipcodes
12/24/2017 Mid-Day Truck 1 10 am - 2 pm 2 0 91352 91504 91505 91506 91502
12/24/2017 Mid-Day Truck 2 10 am - 2 pm 4 1 91405 91406
12/24/2017 Evening Truck 1 2 pm - 4 pm 4 1 91504 91505 91502
When the zipcode is found in 'zipcodes' column, return all 'date1' in the first dropdown and their related 'timeframe' in the second dropdown.
When the order is confirmed, I need those 2 selections added to order_summary, the email confirmation and the dashboard; additionally the column 'count' needs to be updated to +1
When 'count' >= 'slots' the date and timeframe should not show up as a selection anymore, and of course, if date1<curdate it should not display anymore as well.
Because of the nature of the business, the date, timeframe and slots vary day by day so this is not based on a Monday-sunday schedule.
I'd like to be able to have a calendar if possible on dropdown1 to make it easier for my customers to see what weekday they are selecting.
