Custom wordpress plugin to check salary anomaly

I want to create an interface to collect information (Salary Anomaly Checker) that must be either a pop-up or must be prominent. It must collect information that will, on submit send me an email but also on-screen give feedback to the user. The user must enter scale code and salary which will be checked automatically against a pre-programmed set of criteria. The purpose of interface is to tell the user whether he is a candidate for a salary increase in terms of a salary anomaly project. The criteria will be submitted via a back-end interface and should be admin changeable. Example, the mid point for scale 300 is $3350. When user submit information, it will tell user after comparing it to admin programmable data that “According to the information submitted, you are on par with the midpoint for scale (scale code submitted by user), or “According to the information submitted, it appears you are (% percentage) (above/below) the midpoint for scale (scale code sumbited). Ideally I want the user to also register, and the extra information submitted should be captured in database to enable me to contact the user. So, the checker must be accessible to everyone landing on site and if possible pop up on first visit. On second visit it should be accessible via prominent interface. The checker must collect enough information to enable me to contact the user afterwards. It must mail to this address all the collected data, but also store it in database. It should have, on the feedback “According to…..) have a checkbox/radio button asking the user whether he is a member of XYZ (assume XYZ as a criteria). If the answer is yes, I want user to see a form with a request to please registration site, and update latest information to be promptly informed of latest information in workplace. The fields must be stored in database but also mailed to this address. Database must be able to export to Excel. If answer is no, he must receive customized message to say “Join XYZ …….” With an online application form. Form already there, but must be customized. I want the interface to put the data also in a printable form that the user can print. On print it must save and mail the [login to view URL] application form must ask user who employer is. For all Non-ABC (assume ABC as an organization )members an additional part of the form must open up where the form allows for payment. One of the form fields must ask for basis salary. The financial/payment part must calculate 0.475% of basis salary to be paid over monthly to our account (details will be submitted) until time the user cancels it. That option must not be accessible to ABC employees, unless admin activated for individual users. FYI, i have just simple website on twenty-fifteen theme.
