Drupal Open Account Form

Hi, We need a section on our site where we can send clients to register to open a credit account with ourselves (not Drupal user registration) What I need Multi step form collecting data and when the forms steps are complete and if a direct debit option is chosen then the form needs to display a UK direct debit preview echoing some data collected in the multi step form with a submit button, I must be able to edit the preview, once the form is submitted a Confirmation page with message should be displayed echoing some data from the form. When the form is submitted, it should be sent to a customise email address. Thanks
