Dynamic form with table view

I want someone to create a script which includes: 1) A dynamic form (horizontal) that writes the submitted content to my database a) Fields (displayed horizontally in a row): "ID" (text, hidden, auto-increment, unique)*, "Author" (text), "Title" (text), "Format" (dropdown), "Language" (dropdown), "Owner" (email) and "Status" (text, hidden) b) Dynamic buttons: at the end each fields row, a "+" button (to add a new row of fields) and a "x" button (to delete the current row of fields) c) Submit: a "Submit" button to send data to the database (with error message if one or many fields rows are incomplete or contain wrong formatting, example: "Owner" field should be an email address). Once the form is successfully submitted, the script will automatically launch the download of a .XML file containing the submitted values (one per line. If this is too hard to code, this can also be triggered using a "Download .XML File" button on the form confirmation page) 2) A table view of the all the database records (100 per page) a) Columns (with headings): "ID", "Author", "Title", "Format", "Language", "Owner" and "Status" (the "Status" column will only contain one Javascript button called "Dead or Scam" per row. If 10 people click on this button, the record will be deleted from the database. Each person can only click once) b) Search: a dynamic search bar will be placed above the table (when entering search value, the table should be automatically reorganize in front of the person's eyes in order to show the search data) c) Prev/Next buttons: "Next" and "Previous" buttons will be placed underneath the table, allowing visitors to navigate through the records) c) Export: an "Export" button will be placed underneath the table (when clicked, the whole database content will be exported as an CSV file) * : the "ID" field is an auto-increment text field because it contains both letters and digits (example: XXX001, XXX002, XXX003, etc.).
