edit php css template

I need to change templates for 3-5 pages of the software used on my web site to manage payments. This software is called aMember, it's an advanced PHP script that allows to add custom templates.

I need someone that would have the knowledge to create a custom template so I can change pages like

to add a much more professional look and CSS style to the form and content, add the logo correctly, etc. I need it to look very professional, clean, neat and simple.

Those fields are inserted in default [login to view URL] loading variables as explained in the instructions on how templates can be edited are in the link below. We'll need to change order of a couple and make the form look professional, now it looks very amateur and it should change.

I believe there will be like 3-5 pages to edit, the longest one is the link below, then there is the invoice payment invoice, then thankYou page and maybe 1-2 short ones.
