Laravel 5.4 dynamic form creation

I need a laravel 5.4 dynamic profile form written. I will need the database migrations, models, routing, controllers and the blade files for the form input and the show profile blade file. The form will need security and validation. It will be based on the form-model binding in laravel 5.4. It will update multiple tables at once. The tables will be User_id based and will be one to many relationship. The Authentication used will be the built in make:Auth setup. Several sections of the form will need to be dynamic with addable fields as needed. like in this video [login to view URL] Submitting with ajax is an option to keep the form from reloading etc. The form will need an update method that populates the latest info in the form using the same hierarchy as Laravel 5.4 . The form also has a photo a single profile photo that will be uploaded by the user. For the experienced Laravel coder, this is a 1 day project at most. If you have built a form like this before it should only take a couple hours. This is an ASAP project. I need this to be delivered either as a zipped laravel project folder or as a git hub or a live working proof of concept. I will need to verify it meets my requirements before any payment is released.. It does me no good to receive code that doesn't work.
