I need simple form with paypal form and pay button

I need a form where user can enter Name: Email: Phone: comments: Upload images up to 20 images Then a choose package option, user clicks on features he wants, and price changes as features user clicks on. The price change shows on page automatically as user clicks on differnet options. Then when user is ready to check out, he checks out through paypal or enter credit card through paypal. After user pays the email gets sent to Admin with the form user filled out Super simple if you have any experience in this, should take 20 minutes or less if you are familiar with creating paypal form with checkout.
