Webpage for form submission with alerts

I need a webpage built in php and mysql, you can use a framework or reuse a code or similar.
First, the user has to create an account with his email (email confirmation requiered)
After the user is correctly subscribed and logged:
The purpose of the site is that the user enter a form (10 fields) with the option to attach documents and a due date. The sender receive a copy of the form in his email, and an ID number of the form. (lets said is now a "ticket")
After submit the ticket will be in status "alive".
The user (same who submitted the form) have the chance to put the form in "completed" status before the due date,
If the user don't put the "ticket" in "completed" status before the due date, he will receive an email asking to update the status of the form.
If the user don't put the "ticket" in "completed" status an hour after the "due date" the form is going to be sent to a email addres (specified in the same form)
So.
At least seven pages:
1.- Loggin
2.- Account creation
3.- Update Account information
4.- Reset password page
5.- Page for form submission
6.- Page where the user can update the status of the form. (the user can see all his forms)
7.- An admin page (only for user with that permission) where you can see all forms and status of each one.
workflow:
1.- User creates his account
2.- User submit a form
3.- A copy of the submitted form is sent to his email
4.- If the user updates the form to "completed" before due date, workflow ends.
5.- the due date was reached. An email is sent to reminder the user to update the status.
6.- One hour after the due date a copy of the form is sent to two differents emails (these emails where declared in the original form)
