Convert a Template to a Website

Need to convert Excel spreadsheet (100-120 rows) with some customer data entry fields and some pre-recorded values/formulas/lookup fields into the Dynamic Fillable Form (preferably webpage). Should be able save/access/modify the data later if needed and dump it into the PDF document/report. Would like to utilize Google Forms/Docs or some if possible. Main Reasons for Conversion: - Make entering data into the spreadsheet more user friendly and error-proof - Make the form Dynamic - Be able to dump some specific data from the spreadsheets into the template documents. - Be able create/access/share the form on-line. If this conversion will go successfully, will need to convert about 10 more spreadsheets/documents in the next couple months. So most likely this job will lead to long-term project.
