Customize a HTML5 Template

Hello, I'm looking for a Front End Developer to customize a html5 template. The html5 template is well documented and will be provided. (You can find the template and more information about it in attached at the bottom*.) Some design parts needed are included in original template [as you can see in notes included in attached], your responsability is customize 4 pages based on the image mockup I attached. All the pages, but mainy "sheduling page", are open to alternative creative solutions. You can send me a image preview of your proposal and you should be able to do that in html5 as in image sent. Freelancers who send me final project "previews" (desktop+mobile) will be appreciated and will be preferred in final choose. Important: All pages are full responsive and should remain responsive.
