Add PHP calendar to Wordpress blog

Hello

I need to add this PHP calendar ([url removed, login to view]) to my blog with some customizations (color, size, font, a Category Drop-down Filter, and a text link for people to email me their event).

It will look like this on a separate page on my blog:

but again with customization to make it look better and like a part of my blog.

The calendar needs to be installed on an already created page on my blog.

Lastly, there is a current WP calendar on my blog's sidebar that would need to be removed.

I need this done for Tuesday, Dec. 8th at 12 pm in the United States, please.
