Data Entry From Excel To PDF

I have survey results scanned and put into 12 pdf documents. I need the survey results transferred to an excel spreadsheet so that I could analyze it better.

I have started an excel spread sheet which could be used. If you feel you have a better way of organizing the data in excel for my analysis, I am open to ideas.

I attached two of the pdfs and the excel spreadsheet so that you could see what you will have to do.

I need this done by 4 pm EST.
