Create 2 x editable PDF's

This is simple quick job for someone who knows acrobat well.

I have created 2 word documents. 1x 1 page and 1 x 4 pages.. some fields in these are editable..like enter address etc.

I need someone to create 1 x PDF template of these 2 x word documents with editable fields.

other functionality is that the customer needs to save the PDF after entering their details and email back to us.

the PDF's have to replicate exactly the word document layout, text, photos, clean lines etc etc.

I have also created a paypal buy now button that i would like to embed into the PDF

I have attached doc... some editable fields are obvious and the others we can email chat.
