iPhone/HTC App to Record Car Accident Details

I need an experienced iPhone and HTC app developer to design and develop an app for each of the two platforms with a supporting website that will allow:
- a user to see a checklist of what should be done in a road traffic accident
- allow details of road traffic accidents to be recorded
- allow the user to access details of local taxis/towing companies/police/insurance/legal companies for notification purposes
- fully detailed Accident log and history
- unlimited photos with the camera or import from photo library
- preview/send Accident Report with images (PDF format)
Information collected will include:
- driver contact details
- accident type
- location
- witness details
- photos
Apps should have similar but enhanced functions to the following apps:

The developer will need to give a unique look and feel to the app, keeping it simple yet comprehensive and a must have app.
Payment will be released on completion of development and correction of all bugs and errors.
