Discharge card for a Hospital

I need a very simple interface to prepare a discharge card for a hospital. The software should be able to print the discharge card on the letter head. The typical discharge card has around 10 parameters apart from the personal details of the patient. The parameters will be as follows:
- Diagnosis, Treatment done, Treatment advised, Condition of the patient when admitted, Condition of the patient on discharge, Medicines Given, Medicines, Prescribed, Investigations Conducted, Comment. The users will be the doctors who are not very computer savvy. After the data is entered I want it saved in a database which can be viewed as report with a few search parameters or printed in a particular format on a letter head. The medicines can be as many as 20 or as less as 1. A drop down menu to select the name of the medicine and something to write the dosage of the medicine.
