I have my main application up and running, basically want to add a button to my main form which I can do myself, what I need is someone to create another form in VB6, basically need a form where the user fills out some fields, of info and when the submit button is pressed, the information is sent to a file on some website or emailed or something, i dont want an email form though, has to be all in house, so nothing is revealed, info is colelcted, press submit, sends info, says thank you, then user can continue with work. Would like a progress bar displayed showing user that info has been sent. Not looking for anything mega fancy, I can add the xp look etc, all I need from you is the form with the fields, the progress bar connected to it, and it submitting it to some location. Some database work will be involved later. With more work commencing for someone who has work in VB6 and db access. I know I can simply create a simple sendmail form to do the job, but i need someone to tie in the progress bar so it shows the user that the information is being sent. I have also noted that if u create an email form, it can easily be used as spam, i tested this myself, was crazy, so maybe adding an image verification option as well. Down the track I will require the information if possible to be sent to a database, so the main application all it needs to do is sart, then it loads the information from the database located on a website and it appears in the aplication, but for now, this is not needed, but will be required later.

## Deliverables

1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.

2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables):

a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment.

b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request.

3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement).
