Web application development - UI development

I am in the process of developing a web application. The web application will be based on PHP and MySQL technologies for processing and database. A good portion of the application has been developed with a very raw user interface. The need is for some help with a website designer / UI expert to convert some of these raw screens into a more user friendly and real world application. The chosen person or entity will be awarded multiple items of work as portions of the web application are completed and ready for user interface enhancement.

Current work item-

The need is for a slick user interface that will display search results along with a few other elements such as search box etc. Please see attachment for an example of the type of UI that I am seeking. The request is to provide the necessary CSS and jQuery files along with a static page which can then be modified by me to incorporate dynamic content with PHP

Work will be awarded based on the following parameters

1. Quality of work - based on previous examples

2. Speed of delivery

3. Price - this is a self-funded venture, so I am price conscious but there will be a steady stream of work and money as other portions of the application are made ready
