Hi, I'm looking for an App for Iphone (and maybe also Android) to track specific health info for patients.
1. The doctor or nurse will create an account for a patient with associated email address. They will use a web interface to designate one or more diagnoses, one or medications to track, and one or more symptoms to track.
2. The patient will download the app and sign onto the system.
3. The patient will document daily symptoms, use of medications, and other data.
4. This data will be saved onto an online data base with viewers available to the patient or clinican. Data can be sent by email.
5. Notifications will be sent if patients forget to log their data.
I assume there are some online and mobile database solutions that can be used as a template for this project to start from.
