Online HTML form Uploading data in MySQL DB with PHP w/reCAPTHCA and HMTL Signature

I am looking for a freelancer to create an online HTML form. I have the form designed already with html code. However, I desire to add a Drag and Drop section for a picture checking for .jpg, bmp, jfif , gif or bmp, etc... then a Drop and Drag section for multiple files, the ability to use a free HTML signature generator and capture the signature as well as other items above to store in MYSQL db, and use reCAPTCHA - a free service from Google that helps protect websites from spam and abuse. using possible javascripting or PHP that moves data to mysql db for this registration form. Make validation recommendation for fields and storage of the form data into a database. Additionally, I need someone who is experienced in customizing the look and feel of the form to ensure it meets my individual requirements.
The form should be designed to provide a seamless user experience, offering easy navigation and submission of the form’s data. When customer hit the submit button, receives a message data is received.
Experienced coders will be given first preference as this task requires expert coding and a clear understanding of the functionality of the form.
I am looking for a skilled freelancer to complete the job efficiently and to the highest standards. The most important qualifications for this job are attention to detail and great communication. Any prior experience creating online forms is a huge plus. The web site is being hosted at gate.com.
The successful freelancer will be the one who understands the requirements of this project and can complete the task to a satisfactory standard.

The image format must be a “head and shoulders” shot. The image size (in pixel resolution) should good enough to print a 3″ X 4″ photo at 200 dpi: 900 X 1200 pixels will give you the right ratio (3X4) and a good enough size for printing.

Submit Application (button to submit application) fields go into mysql db and print all form fields as described below-
1) Once form is submitted, save form with all the selected fields, and file names attached, Application Terms and Agreement section, Student Acknowledgements section, Contract Acceptance section, along with the written signature, printed name, reCAPTCHA in PDF and send to certain email address. Error checks to ensure sent.
2) When files are uploaded should be to a specified area ( possibly website or other location) Error logging to ensure uploaded.
3) Create the fields in MySQL db and capture data into those fields listed above and as well as the signature, printed name, reCAPTCHA selection. Error checks to ensure uploaded
4) Send all form fields, signature, printed name to MySQL db.
Send email that you are interested in the program.

Design:
- I will give the layout design for the HTML form, so the freelancer will need to implement it accordingly.
