Custom Wordpress/PHP Form - Emails based on form

Need to have custom form created - Brief overview - Form is pretty much what we want for selections….need information from forms to be forwarded to certain email addresses based on the choices in the form. Need it to function similar to form on this page -

Need all choices on one form and have it send based on location and service. Also ability to link 1 or more email addresses based on different answers. Currently using wordpress contact form 7, not sure how customizable this is. Another plugin or custom code would work fine if it functions in wordpress. Can give access to website, send you a shadow copy or have you setup on sample wordpress site.
