This sounds relatively straight forward. The ability to enter a change request, with whatever fields you require. Then the ability to edit, approve, deny, and maybe even delete. Along with user management, role management, and an audit trail. Maybe some reports on top if you need it. I have done similar stuff in the past. While working with J.P. Morgan Chase I designed and developed a password reset solution for their intranet site. They were previously just manually resetting and manually emailing the new passwords. Pretty terrible set up.
