Your assignment is to take one of the below real-life system failures (pick one you are most familiar with), use one of the graphical tools from chapter 4 or 5 (Mind mapping, Ishikawa diagram, Flow chart, or Fault Tree) to generate at least 10 possible causes, then develop a FMA&A document corresponding to those 10+ causes. Computer system: the system has hung up, i.e. no response to keyboard or mouse inputs; it can be rebooted, but it repeatedly does this at seemingly random times. Computer system: the system responds to inputs, but runs no application software and the data files cannot be found; reboot has not been attempted yet. The concern with re-booting is that the system would crash and much evidence for the failiure analysis would be lost. Electrical test equipment (like oscilloscope, logic analyzer, electronics tester, or other small equipment): the system has power, but is no longer working properly. Indicator lights are on, but the system is not responding to any of the buttons or knobs or other inputs. Everything is plugged in and nothing has changed externally; but it's just not doing anything. Vehicle: the engine runs, but the vehicle will not move; it can be put in gear by the user, but will not move; the engine has been turned off and restarted with no change in the response of the vehicle. Provide an original document describing your process, i.e. what you did to generate the information in your project (e.g. if you brainstormed to fill out the tool, if you did some research, if you got a template for your tool from somewhere). Include your name, the date, and this class section on a memo header with the report of your process. Attach the filled-out tool (e.g. filled-out Ishikawa diagram) and a copy of your FMA&A.
