As I'm not good at web based applications, I would do that with an Access database that could be standalone or hosted on a Terminal Server system. The data entry would be made with regular forms and the reports would be generated with excel and mailed to you. To enable remote access (about 5 users) it could be hosted externally on a dedicated server for $30/month or directly at your place at no cost except having a live server and an internet connection running.
