Create and pring custom forms from barcode scanner output

I would like to be able to have the tracking numbers auto appear on the printable form (as per attached) in the Article Numbers area. "Name", "Senders name", "date", and "delivery address" should have a "sticky" default" selection , and a dropdown and manual entry ability. In practice, 1 or more packages are scanned for the SAME receiver. For every scan there will be a default "sender" (Like Amazon) and up to 5 article numbers (US Postal Tracking numbers) can be scanned in. If the "sender" is different (even though the package is to the same person) a NEW form must be filled out. Once there have been 5 article (tracking) numbers a new form must be printed. Idealy the selection of names of recipients, "from" name (like amazon, walmart, etc) should be on a drop down slection at time of scanning. The software should output the forms ready to print (Preferably formatted for double sided print) 6 per page so one A4 size paper (not letter size) can make 6 forms (front and back) You would recommend hardware (scanner ) that could do the job, The form should be printed on paper, so I am not aware of any barcode scanner capable. Since formatting is required to fit on double sided paper, the output would probably feed to MS office compatible or other windows compatible software package to be sent to a standard double sided laser printer
