Code Squeeze Page in MailChimp

I have a squeeze page that was created by a web designer from a gig on fiverr. After going back and forth on the design, he put it together on his software and sent me a jpeg of what the final squeeze page design was going to look like. He then coded the page in html and css and sent me a zip file. I attempted to paste the code into MailChimp and I couldn't get it to work. The designer refuses to do any work outside of fiverr; hence, he won't provide me an email to add him as an author to my MailChimp account. I have attached the zip file with the squeeze page coded and a jpeg of what I want the squeeze page to look like (I had the designer make a few miner adjustments to the font size than what's portrayed in the jpeg attached along with the background pic). I'm looking for someone to paste that code into MailChimp and have it function the way it's supposed to.
